Presented to any Department sworn member, in addition to the Department Emblem of Recognition, wherein the member has continually displayed exemplary appearance while wearing the authorized uniform / equipment items and has maintained a minimum average performance rating of 85 over the last two rating periods.

Presented to any Department sworn or civilian member or citizen who has made a significant impact upon the quality of life within his community by identifying and resolving problems. This award is presented to Department sworn members in the form of a ribbon bar and certificate and to Department civilians and citizens in the form of a “Chicago Alternative Policing Strategy” (CAPS) lapel pin and certificate.

Granted to any Department member who, beginning 01 January 2004, for any thirty-six consecutive calendar month period, has not been on the medical roll, with the exception of IOD. This award is presented to Department sworn members in the form of a ribbon bar and certificate and to Department civilian members in the form of a certificate and lapel pin.

Granted to sworn members who have performed distinguished acts of bravery in the protection of life or property. The awards are presented in the form of a medal and ribbon bar on an annual basis by trustees of the respective award funds. This award is presented to Department sworn members in the form of a ribbon bar, medal, and certificate.
Presented to any group or member of a civic organization, neighborhood group, or law enforcement agency other than the Chicago Police Department by any command staff member.
Granted to to any Department sworn or civilian member for exemplary service, dedication, and leadership. Eligibility for the award is open to any Department member who performs their duties with dedicated professionalism; strives for continuous personal and professional development; demonstrates operational excellence; exhibits professional zeal in the performance of their duties; inspires enthusiasm in their fellow Department members; encourages and instills high expectations in their fellow Department members; maintains accurate knowledge of Department directives; and demonstrates a sense of community awareness. This award is presented in the form of a ribbon bar and certificate.

Granted to any sworn / civilian member on active duty status during the year 2004, a year in which violent crime took a dramatic drop primarily due to the efforts implemented by the Chicago Police Department. This award is in recognition of service of its members and was presented to Department sworn members in the form of a ribbon bar and certificate and to Department civilian members in the form of a lapel pin and certificate.

Presented to a sworn member who has undergone the 40hr Mental Health Crisis Intervention Team State Certification course of instruction. This bar is presented to Department sworn members in the form of the Crisis Intervention Team Bar and certificate.

Granted to any sworn or civilian member on active duty status during the Democratic National Convention held in Chicago from 26 August through 29 August 1996. This award was presented to Department sworn members in the form of a ribbon bar and certificate and to Department civilian members in the form of a lapel pin and certificate.

Granted to any Department sworn or civilian member for an outstanding act or achievement that brings great credit to the Department and involves performance above and beyond that required by the member’s basic assignment. This award is presented to Department sworn members in the form of a ribbon bar and certificate and to Department civilian members in the form of a certificate and lapel pin.
Granted in acknowledgement of an exemplary level of personal achievement in uniform appearance and/or physical fitness by sworn members assigned to uniform duty. Probationary Police Officers are not eligible for this award.
Granted to any Department sworn member displaying exemplary service in identifying, analyzing, or responding to causes, conditions, and problems in an area designated as a Level I or Level II Deployment Operations Center zone. This award is presented in the form of a ribbon bar once a Department member has been awarded five Deployment Operations Center Award Certificates.
Presented to any Department sworn member, in addition to the Department Emblem of Recognition, wherein the member has participated voluntarily, during off-duty hours, in a fitness agility test to determine the members level of physical fitness and has received a passing score on the exam.
Granted to any Department sworn member who has been nominated to the FOP Awards committee in consideration for an act of bravery and/or performing above and beyond the call of duty. This award is presented in the form of a plaque and ribbon bar.

Granted to any Department sworn member who has demonstrated outstanding performance and merits recognition by a Supervisor for a job well done. This award is presented in the form of a certificate.
Granted to any Department sworn member who has demonstrated outstanding performance and has received a minimum of 50 Honorable Mention Awards. This award is presented in the form of a certificate and ribbon bar.
An award of honor conferred upon the star of any deceased sworn member who met death while in the performance of duty, under circumstances that clearly demonstrate courage and dedication in the face of great danger, without regard for personal safety. This award will be a tribute to the member’s courage and dedication.

Granted to a sworn member who, in the opinion of the Hundred Club of Cook County, has demonstrated outstanding courage and heroism in the performance of duty. This award is presented in the form of a medal and ribbon bar.
Granted to a sworn or civilian Department member or civilian members of another governmental city agency whose efforts and participation in a broad multiagency joint operation / event, spanning several days or more, significantly contributed to the overall success of the operation. This award will be presented to Department members in the form of a ribbon bar and certificate and to civilian members and members of outside agencies in the form of a certificate.
Presented to a citizen, security officer, or criminal justice agency due to an act that fulfills or helps fulfill a police purpose in a manner beyond that usually expected of a citizen, security officer, or criminal justice agency. Examples include: help to prevent criminal acts, information that leads to the arrest of a person who has committed a criminal act, aid to Department members in the performance of their duties, demonstration of heroism in the protection of life or property, and assisting the Chicago Police Department in providing police service to the community.

Granted to any Department sworn or civilian member for a successful effort in saving a human life that involved exceptional courage or performance. This award is be presented to Department sworn members in the form of a ribbon bar and certificate and to Department civilian members in the form of a certificate and lapel pin.

An award granted to any Department sworn or civilian member who is currently a member of any branch of the United States Armed Services or was honorably discharged from any branch of the United States Armed Services. This award is also granted in recognition for military service and deployment. This award is presented to Department sworn members in the form of a certificate and ribbon bar with a blue star attachment for military service or certificate and ribbon bar with an oak leaf cluster attachment for military service and deployment. The award is presented to Department civilian members in the form of a certificate and lapel pin; military deployment for civilian members will be recognized in the text of the certificate.
Granted to any Department sworn member who has been honored by a municipal, county, state, or federal governmental agency as a result of exceptional performance of duty, excluding awards issued while in the military, and has brought credit to the Department. The governmental awarding agency will determine the form that their award is to be issued; however, to be considered for authorization to wear on the uniform, it must: 1. be in the form of a ribbon bar that is consistent with the size and shape of current Department award ribbon bars. 2. not conflict in design or color with existing Department award ribbon bars or be so similar in design or color that it would be confused with existing award ribbons.

Granted to any Department sworn member who served in good standing in a Department-recognized pipe band for a minimum of three consecutive years. This award is presented to Department sworn members in the form of a ribbon bar and certificate; to civilian members in the form of a lapel pin and certificate; and members of outside agencies in the form of a lapel pin and a certificate.

Granted to any sworn member who, as a result of accidental causes, has been seriously, critically, or fatally injured while in the performance of police duty. This award will be limited to those cases resulting from an accident (e.g., a traffic accident, heart attack, or other nonviolent incident that occurs in the direct performance of police duty). This award is presented in the form of a medal, ribbon bar, and certificate.

Granted to any sworn member who has been seriously, critically, or fatally injured while in the performance of police duty. In addition, this award may be conferred if injury was averted by wearing body armor. This award will be limited to those cases resulting from attack by an assailant, personal combat, or the performance of an act of valor. This award is presented in the form of a medal, ribbon bar, and certificate.

An award presented annually by the Superintendent of Police to recipients of the Superintendent’s Award of Valor, the Superintendent’s Award of Merit, the Police Blue Star Award, or the Police Blue Shield Award when, in his judgment, the accomplishments or performance of the individual member was so outstanding as to justify additional recognition. This award is presented in the form of a medal, ribbon bar, and certificate.

Granted to all Department members on active-duty status on 4 November 2008, regardless of their duty assignment, for the outstanding police service provided throughout the city during the historic Presidential Rally held in Grant Park on 4 November 2008, an event attended by 240,000 people and resulted in unprecedented national and international attention to the City of Chicago. This award is presented to Department sworn members in the form of a ribbon bar and certificate and to Department civilian members in the form of a certificate and lapel pin.

An award granted to any Department sworn or civilian member or member of the community who shows an exemplary effort to identify, analyze, and successfully respond to causes, conditions, and problems that may lead to crime and neighborhood disorder. This award is presented to Department sworn members in the form of a ribbon bar and certificate and to Department civilians and community members in the form of a CAPS lapel pin and certificate.
An award granted to any sworn member who receives an outside governmental agency award that is awarded by a municipal, county, state or federal agency, excluding awards issued while in the military, as a result of exceptional performance of duty and has brought credit to the Department and: 1. is not issued a ribbon bar by the awarding agency, or 2. the issued ribbon bar conflicts in size or design with current Department ribbon bars. This award is presented to Department sworn members in the form of a ribbon bar.
An award presented annually by the Superintendent of Police in memory of Mayor Richard J. Daley’s outstanding service and dedication to the citizens of Chicago. The award will be conferred upon an exempt member; state, county, or municipal official; or other citizen whose accomplishments or service have brought credit to the City of Chicago and/or the Chicago Police Department. This award will be presented by the Superintendent of Police at an annual awards ceremony.

An award presented to any Department sworn or civilian member or citizen who has made a significant impact on public safety or crime prevention. This award is presented to Department sworn members in the form of a ribbon bar and certificate. Civilian Department members and citizens are presented with this award in the form of a certificate and CAPS lapel pin.
An award presented to any Department sworn or civilian member whose service contributed to an event that has a significant impact upon the historical directions and operations of the Department. This award is presented to Department sworn members in the form of a ribbon bar and certificate and to Department civilian members in the form of a certificate and lapel pin.
An award that was presented by the CTA for help with a CTA elevated train derailment in
the late 1970's.
This award was also presented by the CTA to all members of the Public Transportation Section,
Unit 701, in 1991. The award was presented in recognition of the dramatic decrease in crime
on the CTA Rapid Transit System, in 1990, due to the coordinated efforts of watch, tactical
and canine personnel assigned to the unit.

An award granted to any Department sworn or civilian member for an outstanding accomplishment that has resulted in improved administration, improved operation, or substantial savings in manpower or operational costs wherein the member has gone far beyond the requirements of his normal assignment to contribute to a more effective police service, or for outstanding police work that has brought great credit to the Department in a case of unusual public interest. This award is presented to Department sworn members in the form of a medal, ribbon bar, and certificate and to Department civilian members in the form of a certificate and lapel pin.

An award granted to any sworn member of the Department for an act of outstanding bravery or heroism by which the member has demonstrated in great degree the characteristics of selflessness, personal courage, and devotion to duty. This award is presented in the form of a medal, ribbon bar, and certificate.
An award granted to any sworn member who in the performance of their duty warrants recognition, from the department, for exceptional commitment to the recovery of illegal firearms. The awarded member will have the highest number of incidents during one police period which led to arrestees being charged with firearms-related offenses. This award is presented in the form of a ribbon bar and certificate.
An award granted to any sworn member whom is currently a member of any branch of the United States Armed Services or was honorably discharged from any branch of the United States Armed Services, has made significant contributions to the Chicago Police Department, and is making a continuing contribution to the City of Chicago or State of Illinois through community involvement by means of membership in a civic organization or other comparable method (e.g.: civic organization, school or church involvement, etc.). This award is presented to Department sworn members in the form of a ribbon bar and certificate. This award was developed in conjunction with the Wisconsin State National Guard who will be annually honoring a member of the Wisconsin Army National Guard with the Thomas E. Wortham IV Citizen / Soldier Award.
An award granted to any sworn member who in the performance of their duty warrants recognition, from the department, for excellence in conducting professional traffic stops that result in quality arrests. This award is presented in the form of a ribbon bar and certificate.
